after try using mod for skyrim PS4 dengan menggabungkan file patch dengan base nya
ternyata harus menggunakan tool t42-repackager , Caranya bisa di cari di google atau menonton video dari YT Modden warfare :

Run T42 it it will make folder
you need gengp4 https://github.com/xvortex/ps4-dumper-vtx/releases
you need gengp4 , fake pkg cmd and ext folder copy to bin folder

Copy game base pkg to folder game
and patch update pkg to folder patch

and Run cmd

if error not recognize you must change file pkg name with shorter name

cara nya tidak begitu sulit cuma lama sekali bisa 3 hours proses nya ! (tergantung file size update nya)
And you will need lots of free storage space on your hard drive PC  

Setelah semua berhasil akhirnya saya bisa merasakan skyrim dengan mods di PS4, dan automatic start all mod and new Follower saat memulai game baru... 😀
